Declaration of Waiver

I have voluntarily chosen to participate in Breathwork sessions facilitated by Hailey Lin Harvey. I understand that I am under no obligation to participate and that my involvement is entirely voluntary.

I understand that Breathwork is a personal growth practice intended to enhance quality of life. It is not a substitute for psychotherapy, medical treatment, or professional mental health care, and does not diagnose, prevent, treat, or cure any mental or medical condition.

I acknowledge that I am responsible for my own physical, mental, and emotional well-being, as well as my decisions, actions, and results arising from participation.

Scope of Practice

I understand that Hailey Lin Harvey is not a licensed medical professional or psychotherapist, and that Breathwork sessions are not medically supervised.

I agree that the facilitator(s) shall not be held liable for any actions or inactions, or for any direct or indirect results arising from services provided.

Nature of Breathwork

I understand that Breathwork may involve active breathing techniques and guided practices, and may lead to intense physical, emotional, or psychological experiences.

I acknowledge that I may experience strong emotional or physical responses during or after a session.

I understand that participation may be physically, emotionally, or mentally demanding.

Health Responsibility

I affirm that I am in sufficient physical and mental health to participate.

I understand that it is my responsibility to consult with a physician or qualified healthcare provider if I have any concerns about my ability to safely participate.

If I choose not to seek medical advice prior to participation, I accept full responsibility for that decision and any associated risks.

I understand that it is my responsibility to modify or stop participation at any time if needed based on my own awareness and condition.

I agree to inform the facilitator of any changes to my health that may impact my participation.

Assumption of Risk

I understand that participation in Breathwork carries inherent risks, including but not limited to physical, emotional, or psychological responses.

I knowingly and voluntarily assume all risks associated with participation.

Indemnification

I agree to indemnify and hold harmless Hailey Lin Harvey and any associated representatives, agents, or beneficiaries from and against any and all claims, liabilities, damages, or expenses (including legal fees) arising from my participation.

Release of Liability

In consideration of being permitted to participate, I hereby waive, release, and discharge Hailey Lin Harvey and any associated parties from any and all claims, liabilities, damages, or expenses arising from my participation, whether known or unknown, anticipated or unanticipated.

Acknowledgment

I confirm that I have read and fully understand this Waiver & Release of Liability.

I understand that by agreeing to this document, I am waiving certain legal rights, including the right to bring claims against the facilitator.
